Cerúmen causando otite externa: Cerúmen no fundo do conduto auditivo externo geralmente é decorrente de tentativas frustradas de “limpar os ouvidos com cotonete”. A pessoa acaba empurrando o cerúmen para dentro do canal e pode acabar causando uma otite externa, como se pode ver na pele inflamada abaixo da rolha de cerúmen.

Em algumas pessoas a formação de uma rolha de cerúmen fechando completamente o conduto auditivo externo pode levar depois de muitos anos sem tratamento a formação do colesteatoma de conduto. A pele normalmente se renova e as células mortas são eliminadas espontaneamente para o exterior do conduto. Como ele está bloqueado as células mortas vão se empilhando no interior do conduto e criando um volume crescente que acaba levando a reabsorção óssea de suas paredes, criando um mega-conduto que pode se comunicar com a cavidade da mastoide.

No caso da foto o mecanismo de auto limpeza se reestabeleceu mesmo com o conduto extremamente aumentado em seu tamanho.
Observe-se todo o anulus timpânico (anel fibroso ao redor da membrana timpânica) e espaços aéreos no hipotimpano ( abaixo da membrana timpânica) e na mastoide (posterior a membrana timpânica, no caso o lado é o esquerdo).

In some people the formation of a plug of cerumen completely closing the external auditory canal, after many years without treatment, may to lead to the formation of external cholesteatoma.
Typically renewed skin and dead cells are eliminated spontaneously outwards from the conduit.
As the earwax plug locking the dead cells will be piling into the conduit and creating a growing volume of dead skin cell´s, a mass of skin products, called cholesteatoma, eventually leads to bone resorption of the external auditory canal walls, creating a “mega-canal” that can communicate with the mastoid cavity.
If the video the result of this dease in the physioloy of the exteranl auditory canal in this case has had a self cleaning mechanism reestablished even with the conduit greatly increased in size.
Note the entire tympanic anulus (fibrous ring around the tympanic membrane) and airspaces in hypotymbanon (below the tympanic membrane) and mastoid (posterior tympanic membrane, where the left hand is).

As glândulas ceruminosas se localizam no terço externo do conduto auditivo externo. É normal encontrar-se cerúmen apenas neste local ou mais para fora, para onde é levado espontaneamente pelo mecanismo natural de autolimpeza.

O cerúmen tem uma função protetora e não deve ser removido do interior do conduto.

The ceruminous glands are located in the outer third of the external auditory canal. It is normal to find cerumen only in this location, or more outward, which is eliminated spontaneously by the natural mechanism of self-cleaning.
The cerumen has a protective function and must not be removed from inside the conduit.
