No inicio do filme se observa a parte mais interna da superfície da língua (chamada de base da língua), as papilas gustativas são grandes e formam o relevo característico. Imediatamente se projeta superiormente, no centro da imagem, a Epiglote… ela tem o papel de desviar o fluxo dos alimentos, que vão da boca à garganta, para os lados, em direção ao esôfago, passando pelos “seios piriformes” antes de finalmente penetrar no esôfago na sua viagem ao estomago.

Logo em seguida, no filme, as pregas vocais com sua cor mais clara tomam conta da imagem, elas se aproximam em quase toda a sua extensão durante a fonação, menos em sua parte mais posterior (em cima no vídeo) onde se forma uma “fenda triangular posterior” que é muito frequentemente encontrada em mulheres normais. Os seios piriformes aparecem na parte de cima e dos lados do vídeo.

Diferentemente de uma laringe normal, as pregas vocais desta paciente mostram irregularidades na junção dos seus terços médios e anterior (parte mais baixa do vídeo)…esta irregularidade impede a perfeita aproximação das pregas (coaptação laríngea) durante a fonação e a voz fica rouca.

Na prega vocal direita, a esquerda do vídeo, observa-se alguns vasos sanguíneos que acabam com a “brancura” homogênea da prega vocal deste lado… no mesmo local onde se encontram esses vasos se pode ver uma imagem de cor amarela que corresponde a um pequeno cisto intra-cordal que abaula o bordo livre da prega vocal direita e acaba fazendo um contato excessivo com a prega do outro lado. Esse contato prematuro resulta em hiperceratose (“calo” ou apenas espessaento queratinizado chamado reação contralateral que dificulta ainda mais essa aproximação piora ainda mais a qualidade vocal.

Cyst in the right vocal fold

Differently from a normal larynx, the vocal folds of this patient showed irregularities at the junction of its anterior and middle thirds (lower part of the video) … this irregularity prevents perfect approximation of folds (laryngeal coaptation) during speech and the voice becomes hoarse .
